Want to reserve a specific book kit?

You can reserve a book kit by calling 222-9713 ext. 302, or you can fill out this form. Please keep in mind that it is important to include a backup kit as it is possible the kit you request is already reserved for a conflicting month. Please order kits at least two weeks ahead of needed date.

Instructions for Electronic Form Submission

Once you have completed your reservation form, you can send it to us via e-mail my clicking the "Submit by E-mail" button at the top right of the page. If you are emailing this from your own computer and have email management software (such as Microsoft Outlook) then it will automatically open up a new e-mail with the recipient e-mail and form attached. If you are sending this form from a public computer using a internet mail account, there are a few more steps required:

  1. After clicking "Submit by E-mail", a dialoge box will appear on screen. Chose the second option, "Internet Mail".
  2. A second dialoge box will open asking you to save the form data. Choose a disk location (My Documents or Desktop) and name you can find again.
  3. Open your internet mail site and proceed as normal to send a new message.
  4. Address new message to bethanym@oldhampl.org
  5. Attach form
  6. If using a public computer, please remember to go back and delete the saved form from the hard drive for your protection.
